This list ranks 148 of the largest US nonprofit hospital and health systems by what they paid their CEO, using each system's latest full-year IRS Form 990. Every row links to the return it comes from.

What counts as a hospital system here
Nonprofit hospitals and health systems that file a Form 990, one row per system. A system is ranked by its largest single return, which must report at least $1 billion in total revenue (Part I, line 12); the list is the 150 largest by that measure. Kaiser Foundation Health Plan and HealthPartners are included because they run hospital systems, and Highmark Health as the parent of Allegheny Health Network.
The row for each system is the return that reports the CEO's pay, which is often a parent company rather than the biggest hospital. Revenue is always that return's own; filed revenues are never added together. For 46 systems the ranked return is not the system's largest, and a note on the row names the larger filer. 16 rows are group returns, one Form 990 filed for many affiliates.
Public hospitals, including those owned by state universities or local governments, file no Form 990 and are not here. Hospitals that list the same executive as their system's parent are left out so one person's pay is not listed twice. Not ranked: Vandalia Health (the return that reports its CEO lists under $5 million in revenue, below this site's floor for a page) and Saint Francis Health System (the return that reports its CEO lists under $5 million in revenue, below this site's floor for a page).

Every figure comes from a Form 990 e-file return the organization submitted to the IRS, which the IRS publishes as open data. Each row uses one return and links to it. Nothing is estimated, rounded or combined across years or returns.
- Which return
- The most recent return that covers a full year. Short-year returns, filed when an organization changes its fiscal year, cover only a few months and are not used as the latest year. Where a return was amended, the amended version is used.
- What “total” means
- Part VII columns D, E and F added together: reportable pay from the organization, reportable pay from related organizations, and estimated other compensation. We add them up; each organization's page shows the three amounts separately.
- Timing
- Returns are due up to 10½ months after the fiscal year ends and usually appear in IRS data 12 to 18 months after the year ends, so the latest year here is a year or more old.
- Who counts as CEO
- The person whose filed title names them chief executive (CEO, chief executive officer or executive director). If none is listed, we show the highest-paid person and say so. Totals and percentages labeled as calculated are our arithmetic on figures from one filing.

The list covers nonprofit systems whose largest single return reports at least $1 billion in revenue, one row per system. Hospitals owned by governments or public universities, and for-profit chains, do not file a Form 990, and a hospital that belongs to a larger system is counted under that system's row. Health plans, physician groups and research institutes are left out.
